ESG-CV Homelessness Prevention Assistance

Berean Christian Church Gwinnett has partnered as a coordinating entity with Gwinnett County Government in the ESG-CV Homelessness Prevention Program to administer and distribute COVID-19 relief funds to Gwinnett County residents impacted by COVID-19.

Through the ESG-CV Homelessness Prevention Program, eligible applicants may receive relief funds to cover rent arrears and utilities arrears.  Thank you for applying for ESG-CV Homelessness Prevention Program.

Please read the information below prior to completing the application:

Eligibility For Financial Assistance:

  • Gwinnett County Resident
  • COVID Impact
  • Income Eligibility – below $43,200 per year

Before starting your application, please prepare the following documents for upload:

  • Proof of US Citizenship or Legal Residency – Must have ONE (1) of the following:
    • Valid Driver’s License or State Issued ID
      • Must be valid – not expired
      • Must be over 18
      • Must match address on application
    • Proof of Residency (Utility Bill, Lease, Rent Statement)
      • Must be dated within 30 days
      • Must match address on application
  • Proof of COVID-19 Impact – Must have Proof of COVID Impact documentation:
    • Reduced Hours/Layoff/Furlough/Termination Letter
    • Unemployment Benefits Letter
  • Proof of the Need (Rent, Utilities or both) 
    • Rent Verification of Amount Requested
      • Rent Notice – This can be in the form of email/letter from landlord or verifiable documentation matching the amount requested
      • Lease Agreement or Statement from Landlord showing you are paying rent at the residence listed in the application
      • All notices and statements must be dated less than 30 days
    • Utilities Verification of Amount Requested
      • Bill or Disconnection Notice
      • Must be dated less than 30 days

At the end of the application you will be required to upload all documentation to prove your identity, your income, the loss of income caused by the pandemic. Your application will not be considered until all required fields and documents are submitted.

If you are in need to an application to assist with scanning and uploading documents, download the Adobe Scan app on your mobile device to assist with scanning all documents.  You may also take pictures of documents but all images must be clear and legible.
